Hadracha Forms

Before we enjoy the summer, let's get some paperwork out of the way. In order for you to attend camp, you must complete the following forms. Please upload/complete them through our registration/application platform, CampBrain.

Please note, staff that do not have all forms completed before the start of camp, will not be allowed at camp. This paperwork helps ensure that we have all the information needed to best support you and give the best experience possible this summer.

To complete online: ("Forms" section)

1. Medical Form: This is a health history form. It includes immunization history, dietary restrictions, and more. 

To complete and upload at home: ("Paperwork" Section)

1. Health Insurance card: scan and upload the front and back of your health insurance
2. Participation Agreement: please sign and upload this yourself (if you are under 18, your parent/guardian needs to sign it)
3. If you are under 18 this summer, your parents need to fill out the following: 
1. Medical Form 2 (needs to be signed off by physician)
2. Health Authorization 
3. Chofesh Consent
